Medicare

We accept Medicare Assignment and will automatically file insurance claims for our Medicare patients. Please note that although we accept Assignment, in most instances there will still be a balance due even after your deductible has been met.

Managed Care

Co-pays for Managed Care patients are due and payable at the time of each visit. We will file insurance claims for all Managed Care patients who are members of plans in which we currently participate. See the list of plans below.

Other Insurance or No Insurance

Office Services are payable at the time of the service. You will be provided with an itemized receipt at the end of your visit which contains all of the information your insurer will need to process your claim. Just fill out the personal information requested on the claim form which your insurance company provides to you and mail the form along with the insurance copy of the itemized receipt. All patients are personally responsible for insurance deductibles, uncovered benefits and any outstanding balances determined by your insurer after your claim has been submitted and reviewed.
